Description:
The Geriatric Nurse Navigator RN is a key member of the multidisciplinary care team who functions as an advocate, educator, and facilitator of care for older adult patients. This role is responsible for ensuring all referred patients receive high‑quality, coordinated, and comprehensive services throughout the continuum of care.
The Nurse Navigator collaborates closely with physician leaders and interdisciplinary team members to coordinate care, support clinical pathways, improve outcomes, and advance programmatic goals for geriatric populations.

Description:

This role is responsible for ensuring all referred patients receive high‑quality, coordinated, and comprehensive services throughout the continuum of care.
The Nurse Navigator collaborates closely with physician leaders and interdisciplinary team members to coordinate care, support clinical pathways, improve outcomes, and advance programmatic goals for geriatric populations.
Essential Functions
- Conduct comprehensive assessments addressing physical, psychosocial, functional, environmental, and financial needs of geriatric patients across the continuum of care.
- Coordinate and facilitate timely scheduling of appointments, diagnostic testing, and transitions of care.
- Collaborate with physician leadership and interdisciplinary team members to review clinical history, diagnostics, and treatment options consistent with evidence‑based practice.
- Develop, implement, monitor, and modify individualized care plans using critical thinking and case management principles.
- Communicate plans of care effectively with patients, families, and healthcare team members verbally and in writing.
- Collaborate in quality improvement and performance evaluation activities to improve patient outcomes and processes of care.
- Serve as a patient and family advocate to support informed decision‑making and continuity of care.
- Perform other related duties as assigned.
Education
- Required: Bachelor’s degree in Nursing
- Preferred: Master’s degree in Nursing
- Required:
- Current Registered Nursing License (state of practice)
- CPR or BCLS Certification
- Required:
- Minimum of three (3) years of experience in an acute care setting
- One (1) year of experience in medical, surgical, or radiation oncology (for oncology‑designated specialty)
- Preferred:
- Case management or patient care leadership experience
- Five (5) years of experience in a specialized clinical area
- 1st Shift: Monday – Friday | No weekends or holidays
